Red Sox make 2 roster moves: Tyler O’Neill to IL, outfielder activated

BOSTON — The Red Sox placed Tyler O’Neill on the 7-day injured list Thursday. They have activated Rob Refsnyder from the 10-day IL.
O’Neill has been diagnosed with a mild concussion, according to a source.
O’Neill was injured Monday when he and third baseman Rafael Devers collided in left field going after a 191-foot fly ball. O’Neill received eight stitches for a cut near his eyebrow after he and Devers’ heads collided.
The left fielder/right fielder has been Boston’s most productive hitter so far. He is batting .313 with a .459 on-base percentage and seven homers in 15 games.
Refsnyder began the season on the IL after fracturing his toe during spring training. The 33-year-old right-handed hitting outfielder went 4-for-12 (.333 batting average) with a homer, double, one RBI, three runs, one walk and five strikeouts in four rehab games for Triple-A Worcester.
The Red Sox and Cleveland Guardians will play the final game of their four-game series Thursday. Boston is trying to split the series. Manager Alex Cora is using an interesting lineup with Enmanuel Valdez batting cleanup and Wilyer Abreu batting third.

Houck throws 3-hitter in 2-0 win over Guardians for Boston's 1st CG shutout at Fenway since 2017AP BOSTON (AP) Tanner Houck pitched a three-hitter with nine strikeouts for Boston's first complete-game shutout at Fenway Park in nearly seven years, and the Red Sox beat the Cleveland Guardians 2-0 on Wednesday night. Shout out to Houck for an awesome game. He put on a pitching clinic. It was fun to watch. It's not often that 2 runs of Red Sox offense is enough to win the game.

Red Sox finish homestand 3-7 after 2 more errors, 3 unearned runs in loss

BOSTON — The Red Sox lost 5-4 to the Guardians on Thursday to finish their 10-game homestand with a 3-7 record.
Boston (10-10) fell behind 5-1 because of two errors and three unearned runs. Poor infield defense continues to be a trend since Trevor Story went down with a season-ending shoulder subluxation that required surgery.
Boston has committed a major league-leading 20 errors in 20 games. It has allowed 26 unearned runs.
Reliever Cooper Criswell replaced opener Brennan Bernardino to begin the third inning and allowed four runs, although three runs were unearned.
Cleveland scored an unearned run against Criswell in the third inning to tie it 1-1 after shortstop David Hamilton made a one-out fielding error.
Third baseman Pablo Reyes’ throwing error with no outs in the fifth allowed Ramón Laureano to reach base. It was the beginning of a two-run inning for Cleveland. Lefty Joe Jacques replaced Criswell and allowed an RBI single to Andrés Giménez, making it 3-1 Guardians.
It appeared the Red Sox would escape the fifth inning without allowing another run when José Ramírez grounded to third base with the bases loaded and one out. But the Red Sox failed to turn the double play after Reyes appeared to have difficulty transferring the ball out of his glove before throwing to second base. Ramirez beat out Enmanuel Valdez’s weak throw from second to first base, allowing a run to score to make it 4-1.
Bernardino opens strong
Brennan Bernardino served as the opener for Boston’s bullpen game. He pitched 2 scoreless and hitless innings and allowed one walk while striking out two.
The 32-year-old lefty opened six games the past two seasons. He allowed two runs, seven hits and three walks while striking out 16 in 11 innings.
Boston’s offense
The Red Sox took a 1-0 lead in the first inning. Leadoff hitter Jarren Duran walked and then stole second base. Triston Casas’ infield single put runners at the corners. Duran scored when Wilyer Abreu grounded into a force-out.
Boston rallied for three runs in the sixth after two quick outs to begin the inning. Reese McGuire and Ceddanne Rafaela hit back-to-back singles with two outs. Duran then crushed a two-run single into the triangle in center field.
Duran scored when Guardians reliever Cade Smith balked, making it 5-4.

Red Sox’ Rafael Devers ‘not going to rush’ back from bone bruise

BOSTON — An MRI on Red Sox third baseman Rafael Devers’ left knee revealed he has a bone bruise. He’s unsure yet whether he will avoid an IL stint.
“I don’t know yet. But the thing that I know is like I don’t want to go back too soon,” Devers said through translator Carlos Villoria Benítez after Boston’s 5-4 loss to Cleveland on Thursday. “I’m going to take my time to be ready.”
Devers initially felt knee pain during the seventh inning of Boston’s loss Tuesday. He started at DH on Wednesday but he had difficulty putting pressure down his left leg, which affected his swing. So the Red Sox had him undergo an MRI on Thursday to see if everything was structurally OK.
“When I feel I’m good to go, 100% to play, that’s when I’m going to play,” he said. “I’m not going to rush myself because I don’t want to go back to this same situation. So I’m going to take my time to be ready.”
Devers is relieved it isn’t anything serious.
“At the same time, it’s kind of disappointing that I’ve been having to deal with all these injuries. ... But obviously it’s good news that it’s nothing major,” he said.
Boston (10-10) has had a rash of injuries so far. But Devers said it’s not time to worry with it only being 20 games into the season.
“Every team goes through this obviously,” Devers said. “The press makes it bigger than what it is. But at the same time, we’re not even 50 or 100 games into the season. ... So we still have time to play and have a lot of games in front of us.”

BOSTON -- Soggy and dreary were the fitting elements for the finale of a 10-game homestand in which very little went right for the Red Sox.
When the Sox came home after a 7-3 road trip to open the season, the vibes were good.
But then came a steady wave of errors, unearned runs and injuries. Thursday's 5-4 defeat to the Guardians put Boston at 10-10 as the club goes back on the road for a six-game journey to Pittsburgh and Cleveland that starts Friday night.
Here is an update on the current state of affairs.
The errors -- too many
With errors by third baseman Pablo Reyes and shortstop David Hamilton on Thursday, which led to three unearned runs in a one-run loss, the Sox have 20 miscues in 20 games. There was another play when Reyes couldn't get the ball out of his glove quick enough to start a 5-4-3 double play.
"We need to play better defense," said Red Sox manager Alex Cora. "That's the bottom line. We've just got to keep going. Show up early tomorrow, do early work, keep pushing them. They're good defenders, they are. We're not making plays right now."
The 20 errors lead the Majors. Oakland is second with 17. The unearned run count is 25, which is nine more than the next closest team (the Mets).
It would help the defense if the Red Sox had some of their best players in the lineup instead of being shelved by injuries.
The latest on the injuries
Shortstop Trevor Story is gone for the season after injuring his left shoulder on April 5 and that isn't changing. But having to also play without Rafael Devers in five of the 10 games on the homestand and Tyler O'Neill (the team's hottest hitter out of the gate) for the last three games has stretched the lineup too thin on offense and defense.
Devers was first sidelined by left shoulder pain that forced him out of two games in the first four games of the season and then four consecutive games on this homestand. No sooner had he gotten over the shoulder ailment than he developed a left knee injury, making a play on defense in Tuesday's loss. Initially, there was optimism as Devers was the designated hitter on Wednesday.
But he was back on the bench on Thursday and concerned enough to undergo an MRI, which fortunately only showed a bone bruise.
Will Devers need to go on the injured list?
"Yeah, I don't know yet," said Devers. "But the thing that I know is I don't want to go back too soon and go back to that place. I'm going to take my time to be ready."
That might not be what impatient fans want to hear, but the importance of Devers is great enough to the Red Sox that they need him healthy for the long haul rather than rushing back.
Devers expressed relief that the MRI revealed no structural damage.
"Yeah, obviously that feels good, but at the same time, I'm kind of disappointed that I've been having to deal with all these injuries when I haven't done it before in my career," Devers said.
As for O'Neill, he had a head-on collision with Devers in Monday's game that left him with eight stitches on his forehead. After two days of taking a wait-and-see approach with the outfielder, the Red Sox placed O'Neill on the seven-day injured list, reserved for players with concussions, before Thursday's game. The move was back-dated, and the earliest he can return is Tuesday in Cleveland.
"That's the goal for me, for sure," said O'Neill. "I want to be back out there in uniform with these guys. It's tough being sidelined in this sport, and I've had my fair share, unfortunately, but I've got to get healthy first. I've never had a concussion or anything like this before. Pitching keeping them afloat
Pitching has been the team's overwhelming strength so far, particularly the starting rotation, which has a 1.82 ERA, the best in the Majors. But there are also injuries there.
Nick Pivetta (flexor strain in right elbow) and Garrett Whitlock (left oblique strain) are on the injured list. Pivetta will throw a bullpen session on Saturday and could be back relatively soon. Whitlock's absence isn't expected to be much longer than the 15 days he will serve on the injured list. He is first eligible to return on May 2.
"It's not panic time," said Devers.
"We're 20 games into the season. Every team goes through this obviously. The press makes it bigger than what it is," said Devers. "So we still have time to play and we have a lot of games in front of us."

Devers sat out Thursday's finale of the homestand. At this point, the Red Sox are hopeful Devers can avoid a stint on the 10-day injured list. But the star slugger is taking a wait-and-see approach.
"Yeah, I don't know yet," said Devers. "But the thing that I know is I don't want to go back too soon and go back to that place. I'm going to take my time to be ready. When I feel good and when I feel 100 percent to play, that’s when I’m going to play."
April 18: OF Tyler O'Neill placed on 7-day injured list; OF Rob Refsnyder reinstated from 10-day injured list Three days after O'Neill had a head-on collision with Devers that left him with eight stitches on his forehead, the outfielder was placed on the 7-day injured list retroactive to April 16. The 7-day IL is specifically for players who are experiencing concussion symptoms. Depending on how O'Neill progresses, he could be activated as early as April 23, when the Red Sox open a three-game series in Cleveland. O'Neill said that it's his goal, but he also knows he needs to listen to his body and the training staff.
"So I've had a lot of dizziness, so I'm fighting through that a little bit," O'Neill said. "A lot of like fogginess. First couple of days, I was slow to think. It took me a little bit to recover from that. I'm feeling better now, so I feel like I'm on the right path, sort of through the last stages of lightheadedness. And I'm just a little more fatigued than I'm used to. But I'm thinking better now. And I feel like I'm on the right track."
April 18: LHP Cam Booser will be selected to roster from Triple-A Worcester on Friday A feel-good story will happen this weekend in Pittsburgh, as hard-throwing lefty reliever Booser will be selected to the roster for his first stint in the Major Leagues, sources told MLB.com. Booser, 31, retired from baseball after the 2017 season and became a carpenter. But he made his way back to professional baseball in 2021, advancing himself from the independent Chicago Dogs and then to the Minor League systems of the Diamondbacks and Red Sox. Booser was throwing in the upper 90s for the Red Sox in Spring Training. In four appearances for Worcester, Booser had 15 strikeouts in 6 2/3 innings.
Lefty Joe Jacques, who pitched 1 2/3 innings in Thursday's loss to the Guardians, is a likely candidate to be optioned to Triple-A. The Sox will also have to add Booser to the 40-man roster. Trevor Story, who is out for the season following left shoulder surgery, can be moved from the 10-day injured list to the 60-day IL to open up a spot.
April 18: RHP Nick Pivetta to throw bullpen on Saturday Pivetta will take a big step in his recovery from a right elbow flexor strain when he throws a bullpen session on Saturday. This will be Pivetta's first time off the mound since he was placed on the injured list on April 9.

The Red Sox placed outfielder Tyler O’Neill on the seven-day injured list Thursday with concussion-related symptoms, the move retroactive to Tuesday.
“I’ve had a lot of dizziness. I’m fighting through that a little bit,” O’Neill said after the Sox’ homestand-ending 5-4 loss to the Guardians. “I’ve had a lot of fogginess the first couple of days. I was slow to think. It took me a little bit to recover from that, and I’m feeling better now. So I feel like I’m on the right path. Just working through the last stages of lightheadedness. I’m a little more fatigued than I’m used to, but I’m thinking better now and I think I’m on the right track.”
O’Neill collided with Rafael Devers in Monday’s game against the Guardians on a fly ball. Both were slow to get up, with both teams’ training staffs assisting the pair. O’Neill emerged with a towel over his left eye, which suffered a laceration that required eight stitches.
O’Neill has been a force, batting .313 with a team-leading seven home runs.
After a three-game weekend series in Pittsburgh, the Sox have an offday Monday, and O’Neill would be eligible to return Tuesday against the Guardians in Cleveland.
“Yesterday he had an OK day after the workout,” said manager Alex Cora. “Today, he seems like he has more energy. But you know this is an every-day thing. They’re still going to check on him and we will see.”
Concussions can be challenging to diagnose. The Yankees’ Anthony Rizzo, for example, collided with the Padres’ Fernando Tatis Jr. during a game in late May of last season, but he wasn’t placed on the injured list until Aug. 3, more than two months later. In September, he was shut down for the season.
O’Neill played catch on Thursday and has been riding the bike in the gym so the Sox can monitor his body’s response to an active heart rate.
“I’ve been progressing, thankfully,” O’Neill said. “But just not as quick as they initially hoped. And that’s OK. I feel like I’m on the right path again. It’s just a matter of being patient.”
As of now, O’Neill is scheduled to make the trip to Pittsburgh and Cleveland. The goal, he said, is for him to be activated for Tuesday’s series opener against the Guardians.
“I want to be ready,” said O’Neill. “I want to be back out there and wearing the uniform with these guys. It’s nothing worse than being sidelined in the sport, and I’ve had my fair share, unfortunately. I’ve got to get healthy first. But I think [Tuesday] is a realistic goal.” Devers has bone bruise
Devers, meanwhile, has a bone bruise in his left knee, an MRI revealed.
Devers was the DH on Wednesday night but finished 0 for 4 with three strikeouts following his removal from Tuesday’s game. Devers, who felt pain when he made a play ranging to his right, isn’t sure when he’ll be ready to play again.
“When I feel good to go and 100 percent to play, that’s when I’m going to play,” said Devers through interpreter Carlos Villoria Benitez. “I’m not going to rush myself because I don’t want to go back to the same situation. So I’m going to take my time to get ready to play.”
This season has been a challenge for Devers, who also missed six games because of a sore left shoulder.
“He’s frustrated with the fact that he was doing so good during spring training, and the swing was on point [after] he made some changes. And he was working and all of a sudden, the shoulder and now this knee thing. He’s been healthy throughout his career,” said Cora. “Just like I told Trevor [Story], you put in the work and you’re responsible. I get it, you get frustrated, but I’m not going to be mad at you. I know you want to play.”
It’s uncertain if the bone bruise will require a stint on the IL. Even still, that it’s a bruise and not something worse is a positive.
“Obviously, it feels good,” said Devers. “But at the same time, it’s kind of disappointing that I’ve been having to deal with all these injuries when I have never done it before in my career.” Yoshida sits again
The slumping Masataka Yoshida was out of the lineup for the second straight game. Yoshida is hitting .215/.282/.292 with a .574 OPS and one home run in 71 plate appearances.
“We’re just giving him another day,” said Cora. “He had a rough day two days ago, and I feel like mentally he’ll benefit from one more. So, he’ll be ready for tomorrow.”
Yoshida’s time in the outfield this season has been limited to just one inning. Yoshida is making $18.6 million this season, making him the second-highest-paid player on the roster behind Devers.
Considered a poor defender, the Sox have relegated Yoshida to a DH role. Now, it’s fair to question whether he is fit for that, given that he hasn’t slugged much since last year’s All-Star break. Pivetta progressing

Brayan Bello takes the ball against a starter to be determined for the Pirates. Kutter Crawford will oppose Mitch Keller Saturday. The Sox have yet to announce who will start in place of Garrett Whitlock (oblique) Sunday, with Martin Perez pitching for the Pirates.

Carlos Carrasco pitches solid into the 6th inning, Guardians edge Red Sox 5-4 to improve to 13-6 AP
BOSTON (AP) Carlos Carrasco picked up his first win with Cleveland since 2020 and Andrés Giménez had two RBIs to lead the Guardians to a 5-4 victory over the Boston Red Sox on Thursday.
Ramón Laureano, Josh Naylor and José Ramírez also drove in runs for Cleveland, which took three of four at Fenway Park and improved to a Central Division-best 13-6. The Guardians are 10-3 on the road.
“You want to win every series. Every team talks about it," first-year Guardians manager Stephen Vogt said. "It’s not this new idea, but any time you can go on the road and win a series, especially from a very good team across the way, you feel good about it heading home.”
Carrasco (1-1), who re-signed with the Guardians after spending three seasons with the New York Mets, allowed two runs and four hits in 5 2/3 innings. It was his first win with Cleveland since Sept. 20, 2020.
He left after allowing a two-out single to Reese McGuire in the sixth with Cleveland leading 5-1.
“Trying to be too perfect, which led to the first run," said Carrasco, who was with Cleveland for 11 years before being traded in the 2020 offseason. "After that, I went back and thought the same way I did in the bullpen (for pregame warmups). I just went out there and attacked.
"We’re winning series. That’s important for us.”
Emmanuel Clase pitched a scoreless ninth for his fifth save as the Guardians won three games in a series in Boston for the first time since 2000.
Giménez had two hits while batting leadoff Thursday, and is hitting .293 overall this season.
“He’s the Swiss Army knife of the lineup. He hits all over the place,” Vogt said. “His at-bats since the start of the year have been phenomenal ... clutch hit after clutch hit.”
After starting the season with seven wins in 10 road games, the Red Sox went 3-7 on their first homestand.
Trailing 5-1, Boston cut into Cleveland’s lead behind Jarren Duran’s two-run triple in the Red Sox's three-run sixth inning.

“It sounds repetitive, but we have to play better defense," Red Sox manager Alex Cora said. "That’s the bottom line. We don’t make two routine plays and don’t turn a double play, and we lose by one. They’re good defenders. We’re not making plays right now.”

Guardians: RHP Pedro Avila was added to Cleveland’s major league roster one day after being acquired from San Diego. ... RHP Wes Parsons was optioned to Triple-A Columbus to make room for Avila.
Red Sox: All-Star 3B Rafael Devers wasn’t in the lineup for the series finale against Cleveland. He underwent an MRI before the game to check on the knee soreness that has bothered him in recent days. After the game, Cora said Devers is dealing with a bone bruise. “At least we have an idea of what it is,” Cora said. “He should be fine.” ... OF Tyler O’Neill was placed on the 7-day injured list, retroactive to April 16. Taking his spot on the active roster was OF Rob Refsnyder, who started the season on the injured list (left toe fracture). Refsnyder was used as a pinch hitter in the seventh and played the final two innings in left field.
UP NEXT
Guardians: RHP Triston McKenzie (1-2, 6.23) is scheduled to open the three-game series against Oakland on Friday. McKenzie hasn’t faced the Athletics since the 2022 season. RHP Joe Boyle (1-2, 5.68) is listed as the probable starter for the A’s.
Red Sox: Travel to Pittsburgh for a three-game series that begins Friday. RHP Brayan Bello (2-1, 3.92) will make his fifth start of the season for Boston. The Pirates have yet to announce a starter.
